Back to School Immunizations (7th Grade Requirements)
The following vaccines are required for students entering seventh grade:
1 Tdap
3 Hepatitis B
2 Varicella (Chickenpox)
1 Meningococcal Conjugate- Only Meningococcal Conjugate vaccine given on or after 10 years of age is acceptable for 7th grade school entry.
*Proof of immunity to disease (s) can be accepted in place of vaccination only if a document from a healthcare provider stating the student previously contracted the disease is presented to the school.

Exemptions: Exemption requirements changed as of July 1, 2018. Utah law requires all 7th grade students who previously were exempt from any immunizations to renew the exemption process and provide the school with a new exemption certificate.

(Exemption forms obtained through the completion of the online education module are valid for no less than two years. For example, if a student obtains an exemption form in 6th grade, that student needs to renew his/her exemption form in 8th grade, not seventh grade.)

Exemption Process: Students claiming an exemption to the required vaccinations must have their legally responsible individual complete an on-line educational module (free of charge), or in-person consultation (fee of up to $25) at a local health department, AND provide a copy of the completed form to the school official. Completion of the online educational module or in-person consultation at a local health department must be completed for all types of exemptions including medical.
